Google Forms Connector is a Looker Studio connector that helps to connect Looker Studio with Google Forms and use it as a data source in Looker Studio.

Google Forms is a survey administration software included as part of the free, web-based Google Docs Editors suite offered by Google. The service also includes Google Docs, Google Sheets, Google Slides, Google Drawings, Google Sites, and Google Keep. Google Forms is only available as a web application.
Step 1: Choose the “Google Forms” connector by Jivrus Technologies from Google Looker Studio Partner Gallery
Step 2: Click on the 'AUTHORIZE' button for authorizing your Google account.
Step 3: Once you are authorized to google account, then you need to authorize for third-party service. In this case, Google Forms.
Step 4: Click on the 'AUTHORIZE' button for authorizing your Google Forms account.
Step 5: Click on the 'Allow' button for authorization.
Step 6: Enter the ID or URL of the form you want to use.
To get a Form ID, in Google Form Dashboard, select the form you want to use.
On the website URL, you will find the ID after “/d/”.

Form URL is the Full URL of the website. As in the image, the form URL is : https://docs.google.com/forms/d/1GNymhp70qdjk7P2C75P6jI1mgkMmgdJAAX8PBjZaL_w/edit
Step 7: Click on the 'Connect' button.
Step 8: The available fields from the form are listed.
Step 9: Use this as a data source and continue normally with designing your report in Looker Studio.
Step 10: Click on the 'Create Report' button.
Step 11: You can customize the dashboard as you want.
